Chronic Eye Care Management
Our care coordinators combine their COA knowledge and experience with custom social work training and our data-driven technology platform—developed with over $2M in support from the NEI—to help patients with chronic eye conditions manage their care when they’re not in your clinic. Our Virtual COAs spent over 5,000 hours helping patients between visits over the last year, resulting in a 30% reduction in patient no-shows and a statistically significant reduction in the mean and standard deviation of IOP.
In addition to promoting adherence, our COAs help patients with things like financial assistance programs, prescriptions and refills, insurance issues, transportation to visits, understanding their disease and care plan, and coordinating care. These services qualify for reimbursement by almost all payers, including Medicare, through Principal Care Management codes billed separately from office visits.
